2017 application for Certificate in Sports Studies is now open!
The UP Diliman College of Human Kinetics is now accepting applicants for its Certificate in Sports Studies Program. This round of application only accepts new high school graduates with UPCAT grades of at least 2.800. Applicants with UPCAT grade below 2.800 will not be accepted.
Application Fee (non-refundable)
Total of P500.00 to e paid to Ms. Klem dela Peña, 2nd Floor, CHK Administration Main Office.
Pleae show UPCAT results before payment.
Specific time per test to be released only to applicants during the orientation.

Submission of Application Requirements
The following application requirements should be sent to upchkcolsec@gmail.com, no later than April 21, 2017 (Friday), 5:00pm:- Duly accomplished application form
- Scanned copy* of application fee payment
- Scanned copy* of Form 138 (4th year High School Report Card)
- Scanned copy* of UPCAT Result (with scores of 2.800 or higher)
- Digital copy of 2x2 colored photo. Four (4) 2x2 and four (4) 1x1 colored photos (scanned/colored photocopies are NOT accepted) should also be submitted to Ms. Brigida Geneza, 2nd Floor, CHK Admission Office Annex, no later than April 21, 2017, Friday, 5:00pm. The name should be printed and signed at the back of the photos.
*Original copies will be needed if accepted into the program
Orientation of Applicants
April 28, 2017 (Friday), 4:00pm, Vanguard Building, Room 210
Testing Dates (including panel interview, guidance test, sports readiness test and sports skills test)
May 2, 2017 (Monday) to May 8, 2017 (Monday)Specific time per test to be released only to applicants during the orientation.
Release of Results
May 29, 2017 (Monday), CHK Admission Office- Signing of CHK contracts
- Processing of College Admission Slips
- Processing of University Admission Slips at Office of the University Registrar (OUR)
Failure to attend the orientation and any of the admission tests automatically DISQUALIFIES the applicant from admission.
Applications with incomplete requirements will NOT be processed.
